Below are the forms required to be completed, which you can either:

1. Complete On-Line and submit to us once completed

2. Print out Documents for you to complete manually. or

3. Arrange a time to come into our Office and we can assist you.

 

 

Once the forms have been completed and you arrange a time to come into our Office, you will need to bring 100 points of ID.

This can consist of:

1. Passport

2. Driverts Licence

3. Other

4. Other

The Following form is the Advocates Authority to Act. You can nominate a person or organisation to speak or act on your behalf. We call this a third party representative arrangement. An arrangement can also be in place if you cannot manage your own affairs or payments.
